Below you will find our password protected PDF intake and consent forms. The password will be given to you when scheduling your intake appointment. If you have forgotten the password, please contact your psychologist/therapist via email. If you have difficulty downloading the forms or if you do not have a PDF reader installed on your computer, please go here for a free download. After installation, you will need to go through the set up process and restart the program.

Intake forms should be completed and brought to your first session. These forms will take approximately 30-45 minutes to complete. Forms can be completed by hand, or if you have Adobe Professional installed you can click the Tools tab and select Typewriter mode to type in your responses. Your provider may request that you complete other forms as well. Feel free to contact us if you have any questions.
